The challenge
Serving populations with complex behavioral health, medical and social needs means coordinating outreach, enrollment, services and reporting across payers, providers and members, often in separate tools. Teams needed one secure platform that could identify who needs help, engage them early and keep care moving around the clock.
Our solution
We engineered the product across web and mobile: risk stratification and assessment workflows, outreach and enrollment tooling, programmatic services with remote monitoring, and analytics and reporting for each stakeholder. A member-facing mobile app extends care beyond the clinic. Security and compliance were designed in from the start, in line with healthcare data requirements.
